For this reason, in 2023, it served 92 people in the Galician capital, through the ‘Care program for people with chronic mental illness, homeless or at risk of exclusion’, financed by the City Council of Santiago; of which 11 went through the transition floors that they manage in the city.
With this program, Saúde Mental FEAFES Galicia seeks to promote the integration and engagement of this group in standardized resources, implementing the appropriate actions and services adjusted to their basic needs. To achieve this objective, they will try to establish a coordination network of the health and social resources necessary for the intervention with each participating person.
Attention focused on each person
In the ‘Program to care for people with chronic mental illness, homeless or at risk of exclusion’, we assess the needs of each user to offer them the care and resources they need.
At the beginning, an evaluation is made to establish the itinerary to follow with each person and refer them to the different services of the project, such as: transitional housing, information and guidance, legal advice, leisure, employment, etc. The work itineraries that are created are established by consensus. between the team of professionals and the users.
In addition, the entity supports the team that works on the street and in the transition floors in the detection of people who present symptoms of mental health problems and do not have a diagnosis or do not have treatment. They also seek to establish a solid relationship between people and the health resources they need. FEAFES Galicia also provides support when crisis situations occur, advising the teams with which they work in a network and supporting the users who reside in the transition flats, so that they can meet the objectives set in their personalized inclusion itinerary.
All these actions are carried out in coordination with the entities involved in the care of each person, such as other entities, social services, EVO, FUNGA, health and mental health organizations, Public Administration, etc. People with mental health problems at serious risk of exclusion will improve their quality of life.
