Connected Supports for Personal Autonomy (ACAP) is a service that integrates technological, social and health supports, aimed at people with chronicity, disabilities and/or dependency, who can benefit from an individualized support plan and/or telemonitoring, favoring care integration, patient autonomy and participation. . in the community.
It is considered an asset for health, in which they work in a coordinated manner, together with the entities that develop it (Dfa, FIIS, ATAM), the Aragonese Institute of Social Services, the Aragonese Health Service and the basic social services . .
The project
ACAP is aimed at people:
- In need of monitoring health condition. In it, the entities and HEALTH are coordinated for the registration of basic constants and their telemonitoring.
- Seniors with autonomy in situations of fragility or loneliness.
- Scam disability.
- Scam dependence
or support needs to carry out activities of daily living.
ACAP provides people with comprehensive care, connecting and reinforcing existing community supports with qualified professionals and technological systems, so that people can continue living at home with autonomy and quality of life for as long as possible.
The supports can be one or more of the following, depending on the case, and are generally compatible with others that each patient may already have:
- Technological supports:
oh Non-invasive and intelligent home sensorization.
oh Monitoring and alerts outside the home through a mobile application.
oh Autonomy promotion technicians for support in carrying out activities, accompaniment and participation in the community.
ACAP has no cost for users, is funded by the Recovery, Transformation and Resilience Plan, and will be active at least until March 31, 2025. For the moment (future expansions are expected), it has been implemented in the 3 capitals of the province of Aragon and in the following regions:
• Zaragoza: Central Region and Campo de Belchite.
• Huesca: Hoya de Huesca, Somontano de Barbastro and Los Monegros.
• Teruel: Community of Teruel, Maestrazgo and Bajo Aragón
Doctors, nurses or social workers who identify possible beneficiaries of this project can recommend it as a health asset in OMI-AP, indicating what supports are recommended. The user will sign a consent so that the project technicians can contact him to carry out the assessment and propose a support plan. Applications will be collected by an ACAP technician at each health center. In the case of recommending telemonitoring, the constants and evolution can be consulted in the Electronic Medical Record.
